Raktamokshan
Sira Vyadh is a broad term used for removal of blood. It could be of any type e.g. Venesection, horn, Gourd, leech or scarification (or even a prick)

The principle of Siravyadha procedure is to remove vitiated blood.
Agnikarma
Many people have a false belief that Ayurveda is not useful in management of acute pain. They are unaware about the pain management techniques mentioned in Ayurveda.
Agni karma is a superior para surgical procedure which is highly effective in relieving pain.
Agni means heat & karma means procedure. Agni karma is the procedure in which indirect heat by conduction method is applied over skin surface on the affected part of patient’s body
Viddhakarma and its correlation with Chinese medicine
Chinese and Ayurvedic medicines are contemporary. Indonesia is also related by cultural bonds with India and neighbouring countries since last many centuries and hence the traditional therapeutic medicines developed in this region are having a quite close resemblance with each other. There are definite proofs that various types of exchanges in many respects was a routine practice during that era.

We use Suvarna Shalaka because needle can not be used in diabetic patients. As it doesn’t cause wound, so pus formation is naturally prevented. Of course it is used in pain only. When there is pain of calcaneal spur, then Mrittika Shalaka is used. (Mud or clay probe) In case of corn, heated Loh Shalaka (Iron probe) is used. This probe should not be too pointed.

In practice, one question always comes in our mind is, is there any treatment in Ayurveda to stop pain immediately. No one has shown to us and we have never seen such treatment. But in my professional career of so many years I can assure you that Ayurveda has such treatment to stop the pain immediately.
This treatment of pain relief is described by Sushruta in chapter of Viddha chikitsa.

2 precautions should be taken before doing Viddha or Rakta mokshan. First thing is check if patient gets frightened by seeing blood. If it is so, then we have to perform the procedures carefully in such a way that patient doesn't see blood. Another thing to be asked before Viddha or Agni karma check if patient went into shock after taking any injection or any prick. If such history is present, then both these procedures should be done very carefully.

There is confusion regarding position of penis. Weather it should be erected or not. If modern science is considered, the position of penis is more suitable for Viddha. Specifically, in leukaemia patients, there is symptom of priapism. In such cases, Rakta Mokshan is advised on upper part by taking marking incisions. In calcaneal spur, mud probe (Mrittika Shalaka) is used for Agni karma.
